Please use the following form to request Academic Accommodation Letters from Disability Services. This form is for students who have completed the registration process and have had an initial appointment with a Disability Services Specialist to establish accommodations. Your Academic Accommodation Letters will be emailed to you within 3-5 business days. If you would like paper copies of your Academic Accommodation Letters, come to the Disability Services office after you have received your copies via email.
